How to Request System Access/Create a New User in CELR User

A person can request access to view Clinical Lab Results by first creating a profile within CELR User by doing the following:
  1. Go to the Login screen of the application using the following web address: celr-user

  2. Click on the First-Time user? Click here to sign up link

  3. Enter both your first and last name, and click the NEXT button (If you have been denied, or inactive for 90 days you cannot recover your previous user name. You will need to select "Click here to start filling out the self registration form.")

  4. Enter in all fields required by an asterisk. Please enter a login name that is original to your name. (Do not use a user name that includes your work organization – these WILL NOT be approved i.e. NCSTATELAB)

  5. Please enter in your work information under the primary drop down options.

  6. Enter your work address as Primary. Add an additional address if you have a second office site. Click on the button (>>) to move the address to be saved.

  7. After all demographics are entered check the CELR Clinical & Environmental Lab Reporting box and click REVIEW

  8. You will be shown a copy of the information that was saved. PLEASE MAKE A SCREENSHOT OF YOUR LOGIN INFORMATION FOR RECORD PURPOSES. SECURITY QUESTIONS WILL NOT REGENERTATE AND ARE CASE SENSITIVE. Click the Finish option to complete registration.

  9. You will be notified by email or phone when your access request has been approved by State Lab CLER Administration.

How to Access CELR Lab Results

After your user profile information has been approved by State Lab Administration in CELR User you can access the system to view lab results by doing the following:
  1. Go to the Login screen of the application using the following web address: CELR

  2. Enter your information created by CELR User (Your EIN is your Facilities Federal Tax ID)

  3. Click the Sign in option at the bottom of the screen

Update and Edit User Profile

A user can update his/her profile information (Name, Address, Phone, Email, ...) by doing the following:
  1. Log into the CELR User system at celr-user. (CELR Results is a different program used to search results. It does not manage your profile information)

  2. Click the Update Profile option at the bottom of the screen.

  3. You will see your profile information boxes. Edit any needed information and click Save when complete.

  4. User name cannot be edited once created.

Unlock Account

Please call Customer Service at 919-733-3937 to unlock your account

Reset Password

To reset your password, do the following:
  1. Go to the Login screen of the application using the following web address: celr-user

  2. Enter the information requested - Login name, Security Questions (These are case sensitive)

  3. Click the Submit option at the bottom of the screen.

  4. Then type new desired password and submit. (Must contain at least 8-14 characters, 1 uppercase character, 1 letter, 1 number, and 1 special character. It is case sensitive.)

Specimen Search/Result Download

A user can search for specimen results that are submitted by their location with an EIN and/or EIN suffix only. To view a patient's results from another location, the user would contact the original submitter or call Customer Service at 919-733-3937 for further assistance. FOR NEWBORN – Access to results is not limited by EIN.
  1. Log into CELR Results CELR

  2. You will be prompted to the Search Results upon login.

  3. You will be shown drop down options for "Select a Lab" and "Choose Search Type". Choose the parameters for the test you would like to search.

  4. Enter the information requested.

  5. Click the Search option at the bottom of the screen.

  6. Depending on the search type chosen, you will be shown a list of found items. Click on the folder number to open the file.

  7. You can select multiple files to open through your computer by clicking the checkboxes and then selecting Download.

Download

If you have been given the permission to download data, you will be able to download data from our database to your computer. This data is in a delimited format that can easily be imported into another application such as Microsoft Access or Excel. To Download data, do the following:
  1. Log into CELR Results. CELR

  2. Click the Download option on the black menu bar at the top of the screen

  3. You will be shown a list of download files you have the authority to download. Select the File you wish to download.

  4. If the download needs parameters to extract the data, you will be shown a screen requesting the parameters. Enter the parameters needed and click the Download option to generate results within the set parameters.

  5. You will be asked where to save the file. Fill in the information requested.

Request Additional Site Access

This tab is used for requesting site access to a facility affiliated to you but with a different suffix, AND/OR asking site admins for additional permissions under the comments section (ex: Blood Lead, Newborn) Administrative users for that facility will see your request and determine access rights.
Request site access is not to be used for requesting access to outlying facilities to obtain their results. The user would contact the original submitter or call Customer Service at 919-733-3937 for further assistance. FOR NEWBORN – These are not limited by EIN.
  1. Login to CELR Results CELR

  2. Click the Your Account option on the black menu bar at the top of the screen.

  3. In the tab that opens select Request Site Access

  4. Enter the EIN of your current facility if you need to request additional test permissions from your Administrator. Enter in the comments section what you need; i.e. Blood Lead, Newborn etc. Then click Submit

  5. If you need access to an affiliated Location enter in their EIN and Suffix, enter in the comments section the tests you will need (Blood Lead, Newborn etc.), then click Submit

Administrators

While an NC State Laboratory of Public Health CELR Administrator will monitor approvals, it will be necessary for Site Admins to keep up with requests by accessing the Pending Access Approval tab under Your Account.

Administrator - Create New User

Please have a new user follow the directions in the section: How to Request System Access/Create a New User in CELR User at the top of the page. An Administrator can also perform this on behalf of a new user while logged into CELR by selecting Create New User under the Your Account tab. This link will directly link you to the First Time User page.

Administrator - User Access (CELR Lab Results Website)

Once the user is approved by State Lab Administration, an administrator of the facility may grant access to the user by using the User Access page.
  1. Administrator logs into the system CELR

  2. Click the Your Account option on the black menu bar at the top of the screen

  3. Click the User Access option on the drop down box.

  4. On this page you will see a box with your designated EIN filled in.

  5. Type in the name or full user name of the user you are trying to give access to (They should have already been approved in CELR User by State Lab to be able to see them in the list). Click on their name.

  6. You will be taken to a window showing the user's profile information. Modify the information needed based on what results they will be researching.

  7. Click the Save option to save the information

  8. To verify they were added to your facility go to User List and search the name.

Administrator Modify User Access Rights and Unlock Account

An administrator can set the Labs a user has access to, and give permission for a user to be an administrator, and lock and unlock a user by doing the following. (You can only give lab access to users for labs that you currently have. If you need additional labs you do not see, please call Customer Service at 919-733-3937 for further assistance or request access under Request Additional Access.)
  1. Logs into the system CELR

  2. Click the Your Account option on the black menu bar at the top of the screen and choose User List.

  3. You will see a list of current users that are registered for your location.

  4. Click the user you would like to modify.

  5. You will be shown the users window allowing you to modify the labs to access. Modify the information needed and click Approve.

  6. Users can also be unlocked and locked using this screen. A user will automatically be locked after 60 days of inactivity.

A support ticket will be created and a support technician from the Laboratory will contact you and assist with a resolution..
